To assist us in streamlining your initial visit to our Practice please email the following along with your referral:

• Your Private Health Fund and membership number

• Any related imaging reports

• Blood or urine test results

We triage our referrals based on urgency of the condition.
Once we have received your referral, we will contact you to arrange an appointment.

You will receive an email confirmation with all details of your appointment.

On the day of your appointment you will be asked to fill out a Patient Details form. You may complete this in advance via the link below and email along with your referral.
